Paperwork:
To allow for high quality health care, student-athletes must provide updated medical and insurance documentation annually. This requires student-athletes to enter information in ARMS. Forms are assigned via ARMS in the summer, and will show up for the student-athlete as “tasks to be completed” in ARMS. The sports medicine staff asks that student-athletes complete the forms by August 1 for the following academic year so the staff can review the information and provide medical clearance. Once a student-athlete completes and submits a form, the form will be approved or rejected. If rejected, a comment will be recorded in ARMS to explain what needs to be changed so the form can be resubmitted correctly, and approved.
All documentation must be completed prior to receiving medical clearance, which is required for participation in any team physical activities.

**No student will be permitted to participate in athletics without this information on file in the Anderson University Sports Medicine Department.

**No student will be permitted to participate in athletics without a CURRENT personal insurance policy - ALL International student-athletes are required to purchase the school's international policy with the athletic writer. This is an Anderson University required policy.

Primary Insurance requirements:
All intercollegiate student athletes at Anderson University are required to have maintain a primary health insurance policy that covers injuries and conditions that occur due to participation in intercollegiate athletics. Athletic injury includes any trauma injury, even to the face and teeth. The following insurance will not be accepted as appropriate primary coverage: Medicaid, Samaritan/Medishare, and/or Tricare because these policies tend to make the secondary insurance policy act as a primary policy (see Secondary Insurance Policy Explanation below). Student athletes and/or parents need to verify that this primary insurance covers injuries incurred in all 50 states. If the primary insurance coverage should lapse/discontinue for any reason, the parents/student athlete may be responsible for all medical bills related to their son’s/daughter’s athletic injuries.

Secondary Insurance Policy Explanation

Paperwork to be completed as follows:

  • All documents must be reviewed and signed electronically annually
  • Parents must review and sign electronically where indicated. 
  • The secondary insurance (AG Administrators), is administered by AG Administrators, and the premium is paid by Anderson University.  AG Administrators processes claims and remits payment.  Anderson University only pays the premium, and does not pay medical bills.  The policy includes a $250 disappearing deductible (see the Secondary Insurance Policy Explanation for more information). 
  • The NCAA requires every student-athlete who is prescribed stimulant medication for treatment of ADHD to provide updated records annually in the event that the student-athlete tests positive for a stimulant medication.  The form below should be sent to the prescribing provider to request updated information.

Policy Note:

All student athletes will have a pre-participation physical evaluation (sports physical) performed by Anderson University's team physicians each year. All incoming freshmen and transfers will be scheduled for a sports physical on campus before their first date of competition.
